The volume proposes a methodological reflection on the Individualized Care Plan with the elderly and an operational tool to perfect the ways of acting and communicating in the context of team assistance and the care program, following the principles of the Relational Social Work Method to offer the reader new lenses with which to look at existing tools which are familiar or already used within the organizations where one works. It illustrates a wide repertoire of concrete cases and practical examples and accompanies the reader in effectively identifying different strategies, objectives, sub-objectives and purposes: from the distribution of tasks to the measurement of times, to the role of relational guide, to the sharing of choices and decisions. It also offers useful tips to stimulate creativity in the care project, encouraging reflection, awareness, self-efficacy and fully involving the various actors interested in the well-being of the elderly.
